TDAP Requirements for Rising 6th Graders

Registration hours for new students during the regular school year are Monday-Friday, 8:30 - 11:30 in the Guidance Office. (Students will not be counted in attendance for the day after 11:00.)  During the summer, registration hours are 9:00 - 1:30.  For other times, please call ahead at 819-2855. (Packets may be picked up throughout the day and returned during registration hours the following day.) 

 What do I need to register my child?

  • Proof of address (one of the following:  rental agreement; utility bill; driver's license; or identification card along with voter's registration letter from court house; car registration or letter from employer on their letterhead stating your address. You may also use a voter registration card WITH a Medicaid card in Parent's/legal guardian's name.  The child's name you are registering must be on the Medicaid card.
  • If you are a guardian, you must bring custody papers signed by a judge.
  • Name, address, phone and fax number of the last school child attended.
  • Students from outside the Guilford County School system must complete an Affidavit of Good Standing form and have it notarized.

We also need for you to bring the following documents in order to expedite your child's registration process:

  • birth certificate
  • copy of immunization records
  • copy of last report card and End-of-Grade test scores
  • current copy of IEP if your child is being served in Special Education

*Note to all sixth grade parents - there is a new state immunization requirement for 6th graders, all must have a Tdap booster on or after 5 years of age.  ALL students must have a 2nd MMR.  ( GCS TDAP information )
