Instructions for Salary Verification Forms

For Licensed Employees

Those individuals who have been hired in a licensed position and wish to add previous experience to your NC Educator's License must use the attached verification forms.

Form E - Verification of K-12 Teaching Experience

• This form needs to be completed if you have prior K-12 teaching experience, one for each former employer.
• Part A is used for verifying professional educator experience.
• Part B is used for verifying instructional Teaching Assistant experience.

AND/OR

Form CE - Verification of Post Secondary (College) Educator Experience

AND/OR

Form NE - Verification of Non Teaching Experience

Guidelines for Eligibility of Experience Credit

For Classified Employees

Those individuals who have been hired in a classified position and wish to have previous employment reviewed for possible experience credit must complete the attached "Employment Verification" forms. Please review the specific requirements for verification of previous experience carefully.

Health Certificate

The attached Health Certificate is required of every employee PRIOR to starting employment with the district. A physician must complete this form and you must have a Tuberculosis skin test.

Guilford County Schools will incur the cost of the Health Certificate for all Classified Personnel. If you have been nominated for a classified position, a representative of the Human Resources Office will contact you with more information regarding the specific arrangements for scheduling the Health Exam.

Those individuals seeking employment in a licensed position or as a Substitute, Tutor, or Coach are responsible for obtaining a completed Health Certificate at their own expense.

Those individuals who have not yet established a NC teaching license but completed an approved education program may be required to submit a Form V: Verification of Completion of an Approved Education Program in order to verify eligibility for licensure and employment.
