Food Allergies & Special Diets |
 The needs of your child are very important to us. School Nutrition Services can make accomodations for special diets as ordered by your child's healthcare provider. Please print the Diet Order Form or obtain a copy from the Cafeteria Manager at your school. Diet Order Forms should be completed annually. Please follow the instructions on Page 3 of the Diet Order Form to ensure the information is accurate and complete. Part B must be completed by your physician. |
Return the form to the Cafeteria Manager or School Nurse at your school who will forward a copy to our Registered Dietitian. The manager with will work our Dietitian to make the necessary menu modifications. |
