Kindergarten Registration Requirements

Starting with the 2009-10 school year, state law requires children to turn 5 years old by August 31 to be eligible to enroll in kindergarten.

Parents need to provide the following items to their child’s assigned school in order to register for kindergarten:

  • A certified birth certificate
  • A fully completed kindergarten health assessment report. The form is available at elementary schools and the Guilford County health department, or you may download the file. Click here to download the instructions for the kindergarten health assessment form.
  • A record of your child's immunizations (shots) from a pediatrician or the Guilford County Department of Public Health, including:
    • Diphtheria, Tetanus, acellular Pertussis (DtaP). Five doses required. If fourth dose is after the child's fourth birthday, fifth dose is not required.
    • Inactivated Polio Vaccine or Oral Polio Vaccine (IPV or OPV). Four doses required. If third dose is on or after child's fourth birthday, fourth dose is not required.
    • Measles. Two doses, with first dose on or after child's first birthday.
    • Mumps. One dose on or after the child's first birthday.
    • Rubella. One dose on or after the child's first birthday.
    • Haemophilus Influenza Vaccine (Hib). One dose.
    • Hepatitis B. Three doses. This vaccine is required for all children born on or after July 1, 1994.
    • Varicella. One dose. This vaccine is required for all children born on or after April 1, 2001.
  • Proof of residence showing name and address, such as a utility bill or rental agreement. Telephone or cable television bills are not acceptable. For more information, see below.

Attendance Zones and School Assignment

All GCS students must reside with a parent or legal guardian who is a district resident. To find your child’s assigned school, use the GCS School Locator. Information on attendance zones can be obtained also by contacting the Student Assignment Office at 336-370-8303.


Acceptable Forms of ID for Proof of Residence

  • Gas, oil, water or electric bill with the parent's/legal guardian's name with address in your school zone. Telephone and cable television bill are not acceptable ID.
  • Official lease agreement in the parent's/ legal guardian's name.
  • Driver's license (or State ID card from Department of Motor Vehicles) and voter registration card (go to the Board of Elections Office to register to vote - not the Department of Motor Vehicles. DMV will not give parent documentation of registering to vote.) Driver's license or State ID card must be in parent's/legal guardian's name with address in your school zone.
  • Driver's license (or State ID card) and car registration in the parent's/legal guardian's name and address in your school zone.
  • Driver's license (or State ID card) and letter from employer verifying address. Letter from employer must be on company letterhead.
  • Driver's license (or State ID card) or voter registration card and Medicaid card in the parent's/legal guardian name with address in your school zone.
