1. Please note this will work best on a laptop or desktop instead of a mobile device.  
    2. Save the document as a new file and rename it with the student’s LAST NAME FIRST NAME (ex. SMITH JOHN.doc).  
    3. Please complete the Parent/Guardian and Student sections. We ask you to complete as much as you are comfortable sharing with us. 
    4. Email the completed form following these steps:
      1. Send form as an attachment not a shared link
      2. APPLICATION INFORMATION in the subject area of the email
      3. Send to booner@gcsnc.com (Dr. Rodney L. Boone, Principal)
      4. Submit as soon as possible and no later than Friday, February 25 at 5:00pm
      5. A reply will be provided to acknowledge receipt of the document when processed
