The following forms are available to download: 

Caregiver Affidavit- This Affidavit should be completed for students living in the Guilford County School District, but who are residing with a person who is not the parent or legal guardian. This Affidavit shall be completed by the adult with whom the student is living. The 2025 -26 Caregiver Affidavit is available below to print, complete, be notarized and mail to Student Assignment Office. This form can also be picked up and completed at the Student Assignment Office at 120 Franklin Blvd, Greensboro, NC 27401.  Please call the Student Assignment Office to make an appointment at 336-370-8303

Residency Affidavit- This Affidavit should be completed for families who do not qualify for McKinney-Vento, and do not have the documents accepted for proof of address. This Affidavit shall be completed by the property owner/lessee with whom the family is living. This form is good for one school year and must be completed at the Student Assignment Office at 120 Franklin Blvd, Greensboro, NC 27401.  Please call the Student Assignment Office to make an appointment at 336-370-8303.
