Forms and Affidavits
Caregiver Affidavit
This Affidavit should be completed for students living in the Guilford County School District, but who are residing with a person who is not the parent or legal guardian. This Affidavit shall be completed by the adult with whom the student is living.
The 2026-27 Caregiver Affidavit will be available April 1st.
For the current 2025-26 school year. Please visit the Student Assignment office.
Residency Affidavit
This Affidavit should be completed for families who do not qualify for McKinney-Vento, and do not have the documents accepted for proof of address. This Affidavit shall be completed by the property owner/lessee with whom the family is living.
This form is good for one school year and must be completed in person at the Student Assignment Office at 2703 E Florida St, Greensboro, NC 27401. Please call the Student Assignment Office to make an appointment at 336-370-8303.
Verification Forms
Childcare Verification Form
This form should be submitted to support your request for reassignment submission if it is related to childcare.
Employer Verification Form
This form should be submitted with the Childcare Supervision Form when requesting a reassignment.
