Hello! My name is Kimberly Smith, RN and I am the school nurse for Pilot Elementary. The School Health Program supports Guilford County Schools through case management, health education and consultation in order to remove health related barriers to learning. Healthy students learn better and are better able to reach their academic potential.
If your child has a chronic health condition, please let your child’s teacher or the school nurse know ASAP. Appropriate paperwork and a care plan need to be completed to ensure a plan of care is set in place to care for your child during the school day.
If your child needs to take medication while at school, a medication authorization form will need to be completed and submitted to the school.
Every child that is new to a North Carolina Public School in grades K-12 is required to have a North Carolina Health Assessment Transmittal Form completed by a health care provider and submitted to school within 30 calendar days of the child’s first day of attendance.
