• Hello!  My name is Lindsey Kohser, RN and I am the school nurse currently serving Western Guilford High school. The School Health Program supports Guilford County Schools through case management, health education, and consultation in order to remove health related barriers to learning. Healthy students learn better and are better able to reach their academic potential.

          If your child has a chronic health condition please complete the appropriate care plan and submit the completed care plan to school.

          If your child needs to take medication while at school a medication authorization form will need to be completed and submitted to the school.

          Every child that is new to North Carolina Public School in grades K-12 is required to have a North Carolina Health Assessment Transmittal Form completed by a health care provider and submitted to school within 30 calendar days of the child's first day of attendance.
