• ENROLLMENT GUIDELINES

    Student must reside with a parent or court-appointed legal guardian whose address is within Allen’s district OR has been granted reassignment approval through the Student Assignment Office.

     

    Allen Middle will request school records from previous school. However, we strongly suggest that you bring with you the required withdrawal information from current school (regardless of the student’s grade level) to ensure there are no delays in the enrollment process. Allen Middle reserves the right to put an enrollment on hold until proper documentation is provided.

     

    Summer Enrollment

    To learn what information is needed at the time of enrollment, refer to the category that corresponds to your current status below. Once you have all of the items listed below, you are ready to come to our office and complete Allen’s Enrollment Packet. Our summer office hours are 8:00am-6:00pm Monday through Thursday.

    Enrollment During School

    Category #1:

    NEW TO GCS   DISTRICT / RETURNING TO GCS FROM ANOTHER PUBLIC SCHOOL DISTRICT IN NORTH   CARAOLINA OR ANOTHER STATE

    Category #2:

    TRANSFERRING   TO ALLEN FROM ANOTHER GCS SCHOOL

    Certified Copy of Birth   Certificate – ask previous school to include a copy with withdrawal   paperwork if you do not have a copy/original

    Proof of   Residence (ONE of the following)

    • Utility bill - Current (within the   last 30 days), original gas, oil, water or electric bill in the name of the   parent(s) or court-appointed guardian/custodian. (Telephone and cable   television bills are not acceptable.)

    •Official lease agreement in the name   of the parent(s) or court-appointed guardian/custodian.

     

    •Driver's   license (or State ID card from Department of Motor Vehicles) and voter registration card   (obtain from the Board of Elections Office - not the Department of Motor   Vehicles. DMV will not provide documentation of voter registration). These   items must be in the name of the parent(s) or court-appointed   guardian/custodian.

     

    •Driver's   license (or State ID card) and   car registration in the name of the parent(s) or court-appointed   guardian/custodian.

     

    •Driver's   license (or State ID card) and   letter from employer verifying address of the parent(s) or   court-appointed guardian/custodian. Letter from employer must be on company   letterhead.

     

    •Driver's   license (or State ID card) or voter   registration card and Medicaid   card in the name of the parent(s) or court-appointed guardian/custodian.
